UPLOAD YOUR PHOTO

• Your photo must be a .gif, .jpg or .bmp file, sorry no other format will be accepted.
• Click the Browse button to the left.
• It will open up a window of your hard drive.
• Find the path to the photo you wish to upload, double click on photo.
• Path will be listed in the form field.

Once your personal information is entered and the path to your photo added, submit this form. Please double check to make sure you have listed your breed of dog and you have included your photo. By clicking submit, I verify all the information above is correct and the photo submitted belongs to me. Your IP address will be recorded with this submission.
