The information provided below will only used by the websites of Puppy Shop Inc. Your information will not be sold or distributed in any other way. Unfortunately we are now requiring more information as we have had photos submitted that did not belong to the person submitting the photo.

UPLOAD YOUR PHOTO -

  • Your photo must be a .gif, .jpg or .bmp file, sorry no other format will be accepted. Please make sure your file has a unique name, for example your name, name of your dog etc..
  • Make sure your photo is at least 200 pixels, we will adjust it to fit our format.
  • Click the Browse button to the left.
  • It will open up a window of your hard drive.
  • Find the path to the photo you wish to upload, double click on photo.
  • Path will be listed in the form field.
  • Please do not upload a pictures files who's name has different symbols, languages or characters as we will not be able to use the photo.

Once your personal information is entered and the path to your photo added, submit this form. Please double check to make sure you have listed your breed of dog and you have included your photo. By clicking submit, I verify all the information above is correct and the photo submitted belongs to me. Your IP address will be recorded with this submission.
